fix: handle missing error element in server error responses

Summary

• XMLError.from_response crashed with AttributeError when the server returned valid XML that was missing the expected t:error/t:summary/t:detail elements — the crash was then swallowed and re-raised as NonXMLResponseError, misleadingly implying the response wasn't XML
• Now checks each element for None before accessing .text or .get(), and falls back to surfacing the raw XML content as the error message when the t:error element itself is absent
• Distinguishes "not XML at all" (NonXMLResponseError) from "XML but unexpected structure" (a parseable ServerResponseError with raw content)

Test plan

• Existing XMLError tests continue to pass
• Manually verified with a malformed but valid XML response body
