MySQL was a private-beta feature exposed via both flyctl mysql and
flyctl extensions mysql, both backed by the fly_mysql package. Remove
the package and its command wrapper entirely and unregister both surfaces.

Why? It never went live and is based off the old k8s/percona style
architecture.
