refactor(bayn): structure operational and cycle failures

• Replaces positional operational and cycle failure construction with structured typed inputs.
• Keeps this native stack layer independently reviewable at 977 changed lines.
• Bases directly on codex/bayn-effect-tsgo-optional-apis so GitHub shows only this layer.
