Improve safe live debugging observability

Summary

• add safe, non-invasive DbgEng startup profiling, thread accounting, and offline module timeline reporting
• add direct CoreCLR DAC managed breakpoint support and harden native breakpoint behavior
• fix daemon named-pipe HTTP reads to terminate at the declared Content-Length

Notes

• startup report data remains structured JSON-first, with deterministic tabular output for agents and terminal users
• replay daemon transport validates response length and retains bounded response handling

Validation

• existing workspace validation was completed before the rebase; the rebase itself applied cleanly after preserving ignored generated fixture outputs outside the worktree.
