fix: harden Codex approvals and voice session handoff

Summary

• recognize tall Codex approval prompts when the header and yes choices have scrolled out of the visible tmux pane
• preserve CodexApproval routing so auto-approve sends the documented y hotkey
• re-check live card ownership after slow voice dispatch awaits
• guard resume_card_view with the same carrier lock used by session hand-off so a background session cannot reclaim the active carrier
• cover both regressions with parser, auto-approve, voice-switch, and carrier-race tests
