Guard external_toc_template_path against None so the expander no longer crashes with "PosixPath / NoneType" when a project sets external_toc_path but not the template path. Resolve the template against srcdir and write the expanded TOC to a build-local file instead of mutating the tracked _toc.yml.in, then repoint external_toc_template_path at the generated copy. Add the doxygen_group_toc extension (XML+Breathe sidebar group anchors) and register it from doxygen.setup(), plus tests for both mechanisms.
