feat: close the management planning and delivery loop

• require outcome, acceptance, weekly-cascade, and orphan-task closure contracts
• separate active worksets, workstreams, baselines, effort phases, and evidence-based progress
• add leave/WIP and message publication gates with Base field fallbacks
• add synthetic evals and deterministic regression tests
