Visual issues in visit specific information

When viewing visit specific information, there are visual bugs - "Visit lifestage", "Form label", and "Derived variable explanation" are all showing up incorrectly

Form label is incorrectly showing the readonly radio buttons, without any caption. A separate issue will be created to address this.

Visit lifestage is picking up the Redcap configuration to show as an options field (probably a radio button). It is hidden in the static variable information and set to show in the visit specific information. Should it always be hidden based on the Redcap config for this field? The Redcap data pulled to the database only has entries for this under visit specific information.

Derived variable explanation is also picking up the Redcap configuration, to show in visit specific information if the corr_derived_yn value is set. The Redcap data pulled to the database only has entries for this under visit specific information.

The actual issue isn't that Visit lifestage is showing or hiding incorrectly. It may be that it's showing the internal value rather than the drop down value intended to be shown to the user. Or it may be an artifact of the other issues.

Derived variable explanation issues may actually be artifacts of the other issues.

Revisit when the other issues have been addressed.
