feat: entity-aware SKILL.md rewrite with compass framing, bump v0.1.5

Opening frames alignment as conscience, not a feature. Setup flow
explains the five lenses as dimensions of judgment. Living with
Alignment section on honest use, learning from overrides, escalation
without shame. Fix metadata: declare env vars, version-constrain deps.
