Add JSON output mode for automation

Maintainers should be able to use Maintainer Kit in GitHub Actions and dashboards.

Proposal:

• Add a --json flag
• Support JSON output for issue, pr, release, doctor, and onboard commands
• Keep markdown output as the default

This will make the toolkit easier to integrate into automation workflows.
